SIXTH FORM AGREEMENT Sept 2020 – July 2022 In choosing to be a student in the Sixth Form at The Cotswold School, I agree to: • Attend punctually all timetabled lessons, supervised study sessions, registration / tutorial sessions, assemblies and PSHE sessions. • Work to the best of my ability, both in lessons and for independent study; to hand in set work on time. • Remain on site for all independent learning periods (“free periods”) and make good use of these times to study. • Always sign out when leaving the school site (e.g. at break or lunch time) and sign back in upon my return to school. • Phone or e-mail the school (01451 812946 or sixthform@thecotswoldschool.co.uk) as early as possible on each day of absence due to illness. • Not miss any school days due to holidays (including music festivals) and unavoidable appointments during school time. I will inform my tutor and the Sixth Form Administrator of any planned but unavoidable absence by completing, in advance, a Request for Absence Form. • Act as a positive role model for the younger students, which includes adhering to the School’s behaviour policy and no smoking and drugs policy. • Follow the Sixth Form Dress Code. • Take responsibility for all my belongings, including mobile phones (which can only be used in the Sixth Form Common Room) and take care of the School’s environment, including not leaving any belongings in the Sixth Form Centre overnight. • Recognise that the privilege of being a Cotswold School Sixth Former requires responsibility and commitment, and therefore I will participate as fully as possible in school activities. To support me in achieving success in the Sixth Form I am entitled to receive: • A programme of courses delivered by dedicated, professional teachers, using appropriate facilities and resources. • A variety of activities and experiences in addition to academic study, including opportunities for spiritual, moral, social and cultural development. • Careful monitoring of my progress, including: Ÿagreeing personal targets Ÿreceiving feedback from my teachers on the standard of my work Ÿ receiving pastoral and academic support. • Regular reviews of my progress and opportunities for my parents/guardians to come into school to discuss my progress. • Advice and guidance on the routes I wish to follow beyond school, including a written reference for Higher Education or employment.
