Many community acquired wounds are being documented in the skin band and not the pressure ulcer band Measurements are not recorded on admission and on Mondays. Please ensure you measure the wound on admission and describe it. When in doubt take a photo and document in the pressure ulcer band.ET nurses will correct it if needed. Better to err on the side of caution. Best Practice: If you identify a wound on admission. Have a second nurse assess it and document verified with_____________.
