DROPPED OBJECT – Bolt from Dolly track Incident Description: Dropped Object - Bolt from Dolly Track - Fell 15m to Rig Floor On 6th June 2013 at 04:45 hours, during running in hole logging tools on drill pipes, the slips had been set on one of the stands and driller had started raising the Top Drive to get another stand when a sheared bolt with a nut fell in the red zone. Upon investigation, it was found that it was from the dolly track flange 15m from rig floor. The other piece was removed from the flange and replaced by a similar bolt. All personnel were out of the Red Zone at the time of the incident. Slide no. 7
