Speak Up Against Cancer Speak Up Aims and message • Aims to raise awareness of signs and symptoms of cancer and encourage early diagnosis • When detected early Cancer can be cured, treated, managed and lived with. • Take time to know your body and be aware of changes that persist over time. If a change lasts, SPEAK UP and get it checked by a doctor. • • Cancer screening can save lives, it can detect cancers at a early treatable stage.

Brighter Outlook FREE • One-to-one support. • Specialist exercise coaches who understand and can advise on a safe way to exercise. • Taking into account your individual cancer journey our coaches can offer advice around exercise. • You can attend a ten week physical activity programme with people in a similar situation. This can be a specific circuit classes, or a learn to run course. • Signposting to a chosen activity.

What is Community Navigation? Community Navigation is a social prescribing service that provides access to support services and activities A person centred service that helps people with social and emotional needs, e.g. social isolation, low level depression or anxiety, bereavement, financial difficulties 57% of patients referred are over 55 and many are experiencing anxieties about their medical conditions or wellbeing
How does it work? 1. The person is referred by a primary care health professional e.g. GP, nurse or non-clinical staff 2. Community Navigator meets the person 1-2-1, usually in their own home 3. Initial assessment = guided conversation (1 hour appt.) 4. Offer and discuss a range of options and facilitate onward referrals 5. Following up after agreed amount of time to ensure support 6. Usually 1 or 2 face to face appts. with follow up phone calls. 7. A record of what has been done is sent back to the referrer
