The NHS Long Term Plan Our programme will do a lot of work to make the Long Term Plan happen for people with a learning disability, autism or both. But other partners in the NHS and beyond have much work to do too. www.england.nhs.uk 2
The big commitments in the long term plan In line with the rest of the NHS, we will • Make sure the NHS works in the best way possible . So people can get help more easily, close to where they live, when they need it • Get better at helping people stay well – prevention and reducing health inequalities. • Make care better - improve care quality and outcomes • Support staff better • Improve access to digital care eg. technology and online services • Make best use of tax payers’ money • Support local areas to plan and develop services in line with the long term plan www.england.nhs.uk 3
Some of the things we will work on • Giving children the best start in life • Personalised care – making sure care and support is right for each person as an individual • NHS staff will receive information and training on supporting people with a learning disability, autism or both www.england.nhs.uk 4
Some of the things we will work on • Making sure care in specialist hospitals is good – and people are in hospital for as short a time as possible • • Making sure people who have difficulty with their mental health are only given the medication they need www.england.nhs.uk 5
Tackle causes of preventable deaths Learning Disabilities Mortality Review (LeDeR) Programme • Continued funding for LeDeR • The work will continue to grow • We will identify common themes and learning points - Learning into Action • And provide targeted support to local areas www.england.nhs.uk 6
Tackle causes of preventable deaths Increase annual health checks • We will improve how many people over the age of 14 have an annual health check • We want to make sure that at least 3 out of 4 people who can have an annual health check get one • We will measure how many people have one and how good it was • Health checks for autistic people will be tested out • If successful, they will be extended to other areas www.england.nhs.uk 7
All of NHS to understand needs and work together to improve health and wellbeing • Sustainability and Transformation Partnerships (STPs) and Integrated Care Systems (ICSs) will make sure healthcare providers make reasonable adjustments for people with a learning disability, autism or both • Reasonable adjustments are things such as easy read and longer appointments www.england.nhs.uk 8

Respecting and protecting rights 2. Inclusion and engagement 3. Workforce 4. All of NHS to understand needs and work together to improve health and wellbeing • By 2023/24, there will be digital ‘flags’ on patient electronic records • These will show health staff if a patient has a learning disability or autism • This will help different health services communicate better about people’s needs and wishes www.england.nhs.uk 10
