Defining the Hawaii CoP Leadership T eam Primary CoP Partners Hawaii State Department of Health, Developmental Disabilities Division Hawaii State Council on Developmental Disabilities (DD Council) Identified other stakeholders with similar priorities where existing partnerships could be built upon Department of Human Services, Med-QUEST Division (state Medicaid agency) No Wrong Door (under Department of Health, Executive Office on Aging) Hilopa`a (Family to Family Health Information Center) Center on Disabilities Studies (Hawaii’s UCEDD)
Defining the Hawaii CoP Leadership T eam Initial focus on alignment around Home and Community Based Services Created the Training Hui Multi-systems collaborative partnership Address community awareness and staff development needs around HCBS initiatives statewide Leadership team membership and representation has evolved throughout the process
Laying the contextual framework for HCBS initiatives Needed to secure legislative support for funding to shift HCBS services in new directions Systems had shared goal to generate community buy in around HCBS initiatives HCBS Summit held in December 2016 Created awareness amongst stakeholders of the CMS Final Rule community integration requirements Provided an overview of the transformative changes and new frameworks occurring within HCBS programs for person-centered thinking and supports to family Highlighted the overlap of the HCBS requirements across the different service systems
Building a Foundation of Person Centeredness Contracted with Support Development Associates to provide training in Person Centered Thinking to stakeholders statewide Foundations of Person Centered Organizations Person Centered Practice Co-sponsored and hosted by three (3) state DDD will be launching Person Centered systems (DD Division, No Wrong Door, Med- Organizations training this fall QUEST) DDD will be participating with DD service Multiple sessions held over several days to providers in this endeavor reach statewide audience across 5 islands Commitment to lead by example; walk the Over 300 staff representing 40+ organizations walk, not just talk the talk from the various systems attended live training Cross systems participation is being sessions explored

Challenges Ahead Generating buy-in from staff and stakeholders who are already feeling overwhelmed by other HCBS initiatives Incorporating the unique cultural context of families in Hawaii Getting all state partners to the table; getting agreement around next steps Determining how the LifeCourse framework fits into initiatives outside of the DDD and aligns with other State priorities and projects Coordinating the Hawaii CoP initiatives to coincide with multiple and competing timeframes for implementation across the state systems Ensuring alignment of all initiatives involving Hawaii CoP teams
Redefining the Hawaii CoP Leadership T eam Expanding discussions re: Supporting Families to bring more players to the table Center on Disabilities Studies (Hawaii’s UCEDD) Children with Special Health Needs Branch Division of Vocational Rehabilitation Systems are in agreement that service system silos need to be replaced with integrated supports; however, the specifics of the process are still being defined Redefining leadership roles have changed based on which initiatives are involved Ongoing discussions continue to address how to achieve new levels of integration and how the LifeCourse Framework can be used across various state initiatives
