Other required documents ACAT if relevant CN CMA CN Specialist letters CN Medical Discharge CN summaries from hospital (latest) Pathology results CN Other investigations eg xray, CN CT, MMRI etc Medication Chart CN 6. Scan all documents and email to CN enquiries@resecare.com.au. Required 48 hours prior to consultation 7. Res-e-care will confirm receipt Sarah/CN of information 8. Res-e-care will confirm Sarah/CN appointment schedule 9. Conduct assessment of resident CN and enter data on CeGA online 48 hours prior to consultation 10. Advise GP, patient, family of CN appointment schedule at least 48 hours prior to schedule. Entry in daybook 11. Prepare room for consult CN 12. Prepare patient for consult RN/CN 13. Enter resident ’ s name in CN Telehealth (Res-e-care logbook). Date and sign on appropriate column each time a consult is done. 14. Actual consultation Resident, family, GP, CN, Geriatrician 15. Debriefing session CN, Geriatrician 16. Evaluation forms - resident - family - GP - CN
