Intake What documents are required for intake? Id’s – S.I.N, A.H.C,& treaty status card/picture I.D. Birth certificates, child tax, report cards/attendance. Non-band-residency form maybe required. Picture id for children.
Intake Process Fill out application form, caseplan and era. Client index with the province or interprovincial client index check. What was client’s source of income? If client was working, need to bring in R.O.E. or letter of layoff. school- letter client is no longer receiving a living allowance. E.I – letter stating client is no longer receiving e.i benefits. May have been living with family/friends. Child tax. Maybe separated from spouse or escaping family violence. Referral from other First Nations and copy of caseplan
Categories of client’s NETW-A (medical) or A.I.S.H READY Medical from doctor NETW-B (MULTIPLE BARRIER) Drugs/Alcohol Age Region of high unemployment Low Level of education Worker’s may help clients to address some barriers by making referrals to other agencies There has to be three or more barriers which would impact employability Learners- ETW-A ETW-C
