PREMIUM PAYMENT AND CONDITIONS PREMIUM PAYMENT TERMS AND CONDITIONS • N6,000.00 per annum • Individuals that are not LAPO MFB Clients will REGISTER with the sum of N1,500.00 OR (N1,000 .00 for Account Opening and • N500.00 per month for N500.00 for registration) 12 consecutive months . • Individuals that already hold account with LAPO MFB will only pay for registration. • A limit of liability of N100,000 per enrollee per year • A maximum of 10 outpatient consultations per year • A maximum of 15 days of hospitalization per year. • For monthly payment (N500), defaulters will not have access to Medical Care until outstanding is paid and this is subjected to waiting for another period of 3 months
