Performance Improvement Projects for 2016 LOB PIP Topic/Name Reduce Potentially Preventable Visits to the ED STAR and CHIP for Upper Respiratory Infections: Increase access to and utilization of outpatient care to reduce potentially preventable ED visits due to respiratory tract infections. Improving utilization of well-child visits (W34) CHIP with a PCP during the measurement year. To improve Asthma Management through STAR medication management and reduction of potentially preventable ED visits and inpatient admissions. 5
