Meeting Outcomes  Participants will have focused and reflective conversations on the Community Health Assessment data.  Participants will begin forming strategic issues.

Strategic Issues  Fundamental policy choices or critical challenges that must be addressed in order for a community to achieve its vision  Foundation upon which strategies are developed  Build on the results of the CHA  Review what is truly important from CHA  Most compelling findings  Fundamental choice to be made by the community and local public health system leaders  Can be addressed in a variety of ways  Must be something we can address  Should relate to cross cutting findings from the CHA

Example Strategic Issues  How can health care providers work with institutional partners to address the realities of patients’ lives? Small Group Conversations  What are the common themes through out the assessment results? Think positive, negative, and/or neutral.  Document the evidence of these themes.

More Focused Conversation…  What does this information tell us about where we stand in relation to our vision and values?  What does it suggest we need to do to achieve our vision?  BRAINSTORM!
