  5. NJCAT Overview 5  Evaluates support needs in three main areas: (1) self- care, (2) behavioral, (3) medical  Completed online or over the phone  Reassessment done every five years, more often if needed

  6. NJCAT Overview 6  Results of the NJCAT establish the tier in which an individual is assigned  Five base tiers: A-E  Every tier has a corresponding up-to budget amount that funds needed services

  8. Goods and Services 8 A request for Goods & Services will be reviewed against the following criteria to determine approval:  Need is disability-related  Addresses and identified need  Decreases the need for other services  promotes community inclusion  increases safety in the home

Goods and Services cont. 9 A request for Goods & Services will be reviewed against the following criteria to determine approval:  Not available through another entity;  Fully integrated;  Employment-related;  Does not benefit someone other than the individual; and  Available to the general public and not specifically designed for people with disabilities.

Examples of Goods & Services 10  Microwave oven to assist someone in cooking his/her own meals  Classes within the general public  Classes for those only with disabilities*  Generator  Ceiling tracks for lifting  Activity Fees*  Diapers and wipes (not approved from Medicaid) *Please note that theses examples are not all inclusive of everything that can be funded through this service .

  11. Goods and Services Provider Qualifications 11  All providers must exist primarily to serve the general public  If a provider primarily serves individual with disabilities:  The provider must become a Medicaid / DDD approved provider  Provide services detailed through section 17 of the Supports Program and Community Care Program manuals.  Receive payment through claims submitted to Medicaid.

  12. Goods and Service Approval Process 12 All Goods and Service Requests:  Must be included in ISP  Require Division Approval  Receive a Service Detail Report  Render service as outlined in the prior authorization  Submit invoice through FI for payment
