Diabetes prevalence • It is estimated that over 5 million people in England are at risk of developing Type 2 diabetes • If current trends persist, one in ten people will develop Type 2 diabetes by 2034 • Since 1996 the number of people with diabetes in the UK has more than doubled from 1.4 million to 3.3 million • Type 2 diabetes accounts for at least 90% of all cases of diabetes
Preventing diabetes • The risk of Type 2 diabetes can be reduced significantly by reducing weight and waist size, increasing physical activity and improving diet • Landmark trials in China, Finland, the USA, Japan and India show 30-60% reductions in Type 2 diabetes incidence over 3 years in adults at high risk through intensive behavioural interventions when compared to the comparison group
Healthier You • Healthier You: NHS Diabetes Prevention programme has been developed collaboratively by NHS England, Public Health England and Diabetes UK • Up to 200,000 places will be made available, across the country by 2025
Eligibility criteria • 18 years old and over • Registered with a GP Practice • HbA1c between 42-47 mmol/mol (6.0%-6.4%) or Fasting Plasma Glucose between 5.5-6.9 mmols/l within the last 12 months • Not pregnant • Able to take part in light to moderate physical activity
Service Model Commitment to a 9 month long programme
Initial Assessment 45 minute 1:1 appointment with trained Health Coach • Anthropometric measurements • Smoking status • Wellbeing measure • Goal setting • Signposting onto other services
Core: Healthy Foundations 7 x weekly group session (2 hours each) • Session 1 - Understanding diabetes • Session 2 - Physical activity session • Session 3 - What is a balanced lifestyle? • Session 4 - What is a healthy diet? • Session 5 - Practical solutions for a healthier you • Session 6 - Know your health • Session 7 - Physical activity and 3 month review measurements
Maintenance: Prevention Plus 4 x monthly group sessions (2 hours each) • Session 1-Barriers to change, health values, habits and goals • Session 2- Stress, emotional eating and mindfulness • Session 3- Habitual thoughts, triggers, inner critic and self compassion • Session 4- Gaining control of your health, willpower and review
6 & 9 Month reviews 6 month review delivered within Prevention PLUS 3 9 month review delivered as 45 minute 1:1 appointment • Re-test of anthropometric measurements and lifestyle scores • Evaluation of progress • Evaluation of service • Signposting of other services • Goal setting and review
