8 Take a Healthy Step - 2017 CHANGES • Includes the option to choose either: • UPMC Health Plan Premium Network PPO, or • UPMC Health Plan Standard Network HMO (new option) • Both plan options have deductibles & out-of-pocket (OOP) limits that have increased from 2016 levels • Both options offer Health Savings Account (Keystone Federal UM Credit Union) or Health Reimbursement Arrangement (UPMC) OPEN ENROLLMENT: October 1 – November 30, 2016

10 Premium Network PPO Option • Same structure as 2016 with increased deductibles and out- of-pocket maximum • PCP not required (but encouraged) • No referrals needed for specialist visits
11 Standard Network HMO Option • HMO option has lower deductible, however you MUST: • Select your Primary Care Physician (PCP) and PCP# • The PCP & PCP# must be included on your enrollment application. • Receive referrals from your PCP to see any specialists • (excluding OB-GYN, mental health providers and Pediatricians) • Verify that your providers are in the HMO Standard Network, which is more limited than the PPO network. • Accept that there is ZERO out of network Coverage. You will pay in entirety for any services out of the HMO network unless it is deemed an absolute medical emergency.

18 UPMC enrollment application notes • UPMC Enrollment Deadline is NOVEMBER 30, 2016. • If you do not send in your enrollment application by November 30, you will default to the PPO, Health Reimbursement Arrangement, with larger network and higher deductible. • Our enrollment file will be sent to UPMC on December 1st, you will not be permitted late enrollment. • You do not need to check Vision and/or Dental
19 UPMC enrollment application notes • If you are currently on the PPO plan and wish to continue on the PPO plan for 2017 AND you are not adding/removing family members, or changing from HSA to HRA, you can bypass the enrollment application and send us notification in writing with your election to remain on the PPO plan in 2017. • Email sent to Kathleen or Alexis is acceptable, you can also fax or mail your confirmation to the Conference Center. • If you are enrolled in an HSA in 2016 and turned age 65 in 2016, you must enroll in an HRA in 2017 as you are no longer eligible for an HSA. • HMO plan requires enrollment application.
20 Health Savings Account • Not all members are eligible. Eligibility is limited to those who: • Are covered under a high deductible health plan • Have no other insurance coverage • Are not enrolled in Medicare • Not claimed as a dependent on someone else’s tax return

24 Health Insurance Changes: Vision The 2017 vision provider is EyeMed Vision Care, Insight Network. Before January 1, 2017, you will receive a packet with ID cards and additional information. Please note: you will be automatically enrolled in EyeMed by your Benefits Team. More information to follow.

26 Final Thoughts… • Fill your prescriptions (extra months if possible) before January 1, 2017 • If you’ve reached your deductible in 2016, try to have any medical procedures/exams completed before January 1, 2017 • Be a smart healthcare consumer • Take advantage of the programs UPMC provides • Continue contributing to your HSA • Pay attention to Friday UPMC email tips

We have 106 members who have lowered blood pressure and/or lowered cholesterol and many who have lost weight! Lets keep the momentum going!
