Overview of the CCPD Grants Program • By statute, the CCPD Program funds competitive grants to support prevention, early intervention and treatment of cancer, cardiovascular and pulmonary diseases. • The Program is administered by the Prevention S ervices Division of the Colorado Department of Public Health and Environment. • Oversight of the Program is provided by a 16-member Review Committee and the Colorado S tate Board of Health. • The CCPD Review Committee sets priorities of the Program, in cluding the strategies to be implemented for fiscal years 2019- 2021. 3

Negotiation Process - Purpose Purpose Modify what was submitted in grant applications based on specific: • guidance, • feedback, and • recommended or required changes from application reviewers and CDPHE staff to develop final: • scopes of work (multi-year), • budgets (1 year), and • implementation plans (1 year). 7
Negotiation Process - Approach For FY19, the CCPD Grants Program is using a new approach to implement multi-year contracts with associated one-year implementation plans. The goal is to reduce time and burden for grantees and CDPHE staff, and provide more flexibility for grantees. The key documents that will be negotiated and included in the formal contract are: • A multi-year high-level S OW (FY19-21: July 1, 2018 – June 30, 2021), and • An annual budget (to be negotiated each S pring). Negotiation Process - Approach This new process for the program will involve using Google folders to share documents, resources, and drafts throughout the negotiation process. Grantees and CCPD staff will use suggesting mode to make edits and comments in the documents, and email each other with updates. If you (or your agency) are unable t o access or is unfamiliar wit h Google drive, please let us know as soon as possible so we can accommodat e your needs. 9

Multi-Year High-Level SOW • S tandard content for each of the funded strategies and optional interventions. • Customized for each grantee based on included strategies and interventions for the three-year funding cycle that the grantee will be funded to work on. • Do not include grantee proj ect-specific details, as this will be included in the implementation plan. • Ensure the content is accurate and consistent with your proposed scope of proj ect. • Please note, the content should reflect the proj ect that is approved for funding and being negotiated, and therefore may be modified from what was submitted in the application. 12
FY19 Implementation Plan • A template or draft will be provided and required for use. • The template is the same format and structure as the work/ implementation plan completed during the application process. • The template will be based on the strategies and interventions each grantee is funded for. • The document details the work and describes the specific activities, responsible parties, timelines, and outcomes unique to the grant proj ect. • Include all proj ect work and activities for one year, FY19. • A new implementation plan will be developed annually. 13
FY19 Budget • Prepare a detailed budget, not to exceed your agency’ s recommended amount for FY19 (July 1, 2018 – June 30, 2019), consistent with the proposed implementation plan. The budget should include costs for one year only. • A budget template will be provided and required for use. • The template is the same format and structure as the budget completed during the application process. • Additional guidance on each budget category, indirects, required and unallowed expenses, etc. are included in the negotiation guidance. • Matching funds are not required for this grant program. Do not include match or in-kind contributions in the budget template. 14

Next Steps CCPD staff will schedule time with grantees to discuss feedback on applications and negotiations (1/ 29 - 2/ 7/ 18) 1/ 29/ 18, grantees will receive an email with a link to a Google folder containing: • feedback and budget amount • detailed guidance • templates • required documents • other resources LPHAs please review County Board Calendars for May and June to be added to the meeting agenda CCPD staff will seek final BOH approval for grants in March 16
