MPW FOG R EMOVAL P ROGRAM G OALS • Ability for MPW to easily monitor all interceptors within our system • Eliminate sewer system overflows related to FOG • Reduce the amount of oil & grease discharged into wastewater collection system • Reduce maintenance costs for the wastewater collection system related to FOG

FOG I NTERCEPTOR D ISCHARGE P ERMIT • Required by all food service establishments that prepare, process or serves food, to include the following: • New construction • Expansions or major changes in operation • Changes of ownership / occupancy • Required to be renewed annually. • Customers required to have a contract (1-yr min) with a Hauler as part of permit submittal and annual renewal.  It is a violation for a generator to discharge through an interceptor without an approved FOG Interceptor Discharge Permit

FOG I NTERCEPTOR D ISCHARGE P ERMIT • Requirements for New Construction / Change in Ownership: • FOG Program Initiation Fee • Submission of Permit Application by Generator • Submission of Annual Grease Hauling Agreement 1. Customer name, address, telephone number of facility 2. Primary & secondary contact for the facility 3. Grease hauler name and contact information 4. Disposal name/location 5. Grease interceptor(s) size and location, including diagram if needed 6. Frequency of cleaning & inspection required 7. Signed and dated but authorized representatives indicating acceptance of the terms of the contract 8. Contain a statement of the duration of the contract, to be not less than one year, or for the duration of the operation of the facility, whichever is less.

C LEANING R EQUIREMENTS • Grease interceptor must be cleaned and inspected at a minimum frequency of every 90 days, or as determined by MPW • Combined fats, oils, grease & solids shall not exceed 25% of the total hydraulic depth of the equipment • Requests for less frequent cleaning must be submitted for in writing to MPW along with testing required by MPW (internal traps) • Cleaning Procedure: • Evacuate all contents; floating materials, bottom sludges, solids • After complete evacuation, the walls, tops, bottoms to be thoroughly scraped and residue removed • Interceptor should then be washed down and remaining residue removed • Generator shall inspect the interceptor and then personally sign the manifest • Haulers shall provide Manifest Report on behalf of the Generator to MPW within 14 days of interceptor cleaning/inspection. • Generator shall maintain a Maintenance Log containing Manifest Reports for the previous two (2) years on-site and be readily accessible.

P ERMIT R ENEWAL • Renewed annually, or upon a significant change in operation, facility expansion, remodeling, change of ownership, or new grease hauler contract • Generator must submit for renewal of the FOG Interceptor Discharge Permit 30-days prior to the expiration of the current permit. • To be included with renewal application: • Annual Grease Hauling Agreement Summary Form with updated, signed contract • Annual Interceptor Inspection Report & photos to be performed by the Hauler • Annual Renewal Fee

  12. FOG P ROGRAM F EES The below FOG Program Fees have not been approved by MPW Commissioners and are subject to change. FOG Program Initiation Fee: $400.00 • One time fee charged for any new grease trap added to the program or for a change in ownership or operation. • Recovers MPW cost for new account set up and reviewing grease trap for acceptance into the program. • All current grease trap customers with traps installed prior to July 1 st , 2015 will be exempt. Annual FOG Interceptor Discharge Permit Renewal: $200.00 • Submitted with Annual Permit Renewal Application • Charged to review compliance manifests, provide customer assistance and update account information. MPW Deficiency Inspection: $75.00 • Inspection by MPW due to a deficiency noted in Hauler’s manifest • If deficiency is not corrected within 60 days: • FOG Permit revoked and customer must re-apply to include Permit Renewal Fee • If deficiency is not corrected within 90 days: • Customer is removed from FOG Program and must pay Initiation Fee to rejoin or service may be disconnected at MPW’s discretion.

E XISTING G REASE T RAP C USTOMERS . . . • All current grease trap customers with traps installed prior to July 1 st , 2015 will be “grandfathered” into the new FOG Program, and will not be assessed the FOG Program Initiation Fee. • You will be responsible for following the new FOG Program Guidelines at your next scheduled cleaning after July 1 st , and are required to submit the following within 30 days of the scheduled cleaning: • FOG Interceptor Discharge Permit Application *Initiation Fee Waived* • Annual Grease Hauling Agreement with signed contract • Annual Interceptor Inspection Report & photos (performed by hauler) • Annual Renewal Fee Existing customers who have not enrolled in the new FOG Program by Friday, October 30th, will be required to enroll as a new customer and will be responsible for all applicable program fees.

Summary • FOG Interceptor Discharge Permit • Permit renewed annually • Customer responsible to have a Hauler contract, and renewed annually • Hauler to conduct annual inspection to include photos • Cleaning Procedures • Hauler responsible for Manifest/Inspection Report and submittal to MPW • MPW Goals: • Ability for MPW to easily monitor all interceptors within our system • Eliminate sewer system overflows related to FOG • Reduce the amount of oil & grease discharged into wastewater collection system • Reduce maintenance costs for the wastewater collection system related to FOG
