OEBB Open Enrollment 2020-21 Plan Year  Open Enrollment ends September 4 & is required for New Employees  Double Coverage Surcharge to begin October 1, 2020  $5 per month if dependent on your plan and as a subscriber on their own OEBB or PEBB plan  District email will be sent to each of you with OEBB rate sheet and cover letter  OEBB Open Enrollment Booklets are electronic PDFs only – no hard copies available this year from OEBB  Links to all resources are available on the Business Services Health & Wellness webpage  Will be posting some google meet Q&A sessions for staff

Your Medical Plan  Moda Medical Plan 5 integrated with the Group HRA plan which provides a net benefit of $500/$1,500 deductible per person/family and maximum out-of- pocket per person/family of $3,300/$9,900 with coordinated care and in- network providers. Members will pay co-insurance of 25% under the plan. The underlying medical plan, is shown below with its deductibles and maximum out- of-pocket costs. Moda Medical Plan 5 Deductibles and Maximum Out-of-Pocket  In-network Coordinated Care Member pays $2,000/$6,300 deductible per person/family and maximum out-of-pocket per person/family of $6,800/$15,800  In-network Non-Coordinated Care Member pays $2,100/$6,300 deductible per person/family and maximum out-of-pocket per person/family of $7,200/$15,800  Of the $1,594 total premium for health insurance, the District pays $1,477/mo. and you pay $117/mo. pre-tax. (@1.0 FTE) Part time licensed have a prorated District contribution  Premiums for your employee group are based on a composite rate regardless of how many are in your family.  HRA VEBA of $300/mo. is available with eligible other group health coverage

  7. New Group HRA TPA  Group HRA TPA NueSynergy will process 2019-20 Plan Year reimbursements for the EOBs for services during plan year ending September 30, 2020  New Group TPA will begin October 1, 2020  More to come on enrollment process…

  8. Your Dental Plan Moda Delta Dental Premier Plan 5  $1,700 Benefit Maximum with $50 Deductible  Incentive care plan. As long as you visit the dentist at once during the year the level of benefit for certain services will increase the following year (up to a maximum of 100%)  Late enrollees subject to 12-month waiting period for all services other than preventative care  Delta Dental Premier Network

Your Vision Plan Moda Quartz Vision Plan  Benefit runs on the plan year basis (October 1 – September 30)  Benefit maximum of $250 includes exam and hardware  Covers contacts or one pair of lenses every plan year  Covers one pair of frames every two years for members 17 or older  Late enrollees are subject to a 12-month waiting period for lenses and frames

Long Term Disability Insurance & Optional Short Term Disability, Life, & AD&D Standard Insurance Company  Mandatory enrollment Long Term Disability Plan 17  90 days benefit waiting period  Benefit percentage 60% of monthly salary up to $8,000/mo.  Premium rate $0.00353 x monthly salary, employee paid  Optional Short Term Disability Plan 16  30 days benefit waiting period. 90 day benefit duration  Benefit percentage 60% of monthly salary up to $1,500/week  Premium rate $0.00187 x monthly salary, employee paid  Optional coverage for Life and/or AD&D  You, Spouse/domestic partner, Children

Licensed Educators 403(b) Benefits Deferred Compensation  For new licensed educators, the annual 403(b) District contribution is $50 per month with no employee match requirement.  You must select a 403(b) provider and complete the salary reduction agreement before District contributions can begin.  Employee 403(b) or 457(b) deferred compensation plans available through Carruth Compliance Consulting http://www.ncompliance.com/guest_employees.aspx?EmployerID =130
