  3. Safety Moment

  4. B-Double Trailer Fire- December 2017 • 4:51 am : Prowest Logistics Driver departed CS4 bound for CS3 and then CS2 • 9:05 am : departed for CS 2 • 10:30am: As driver approached North West Coastal Highway from an unsealed road he pulled over and “knocked” all the wheels to remove dust (usual practice) as well as check all wheels and tyres • 11:01am: Driver heard tyre blow out, and pulled over safely. Drivers found tyre was on fire and attempted to extinguish but fire grew too strong and too quick. They attempted to separate trailer and prime mover (could only separate prime mover) • 11:31am: fire has engulfed both trailers, all drivers are safe and acceptable distance from fire, await for DFS Onslow • 12:26pm: fire has burned almost out, drivers able to put out small spot fires • 1:15 pm: DFES Onslow Arrived • 3:35 pm: DFES extinguished the fire and declared area safe

  5. Outcomes • That even with the best systems and processes in place that accidents happen • Training and proper equipment minimised the impact of what could have resulted from the accident (bush fires from spot fires) • Training also ensured that there was minimal risk to personnel and environment • DFES commended drivers on controlling traffic and using the extinguisher to put out spot fires
