  8. When is it happening? Affinity Care • Monday 15 th April 2019 – When our clinical system provider will start to merge the two clinical systems together – We will close normal clinics at 3pm to allow this – Emergency cover will be in place via the practice telephone system

  9. When is it happening? Affinity Care • Tuesday 16 th April 2019 – We will reopen at 8:00am as normal – You will still be able to ring us using the normal practice telephone numbers – We have on-site technical support planned for that day – Essentially, it’s “Business as Usual” !!

  10. Shipley Medical Practice Affinity Care • We will be known as Shipley Medical Practice • We will continue to work from both buildings – Shipley Health Centre, Alexandra Road – Westcliffe Health Centre, Westcliffe Road

  11. Website Affinity Care • Our ‘new’ website will be www.shipleymedicalpractice.nhs.uk If patients access the Westcliffe website there will be a redirect for a period of time, taking them to the above site

Affinity Care • GP forward view 2016 - widening workforce. • Care Navigators will be pivotal to navigate to the extended primary care team and local services • Patients receive the right care , by the right professional , at the right time and in the right place (locality) • This means one visit, not two or three or more for the patient • Improved job satisfaction for frontline staff
