Adding HLA DQA1 Unacceptable Antigen Equivalences Table Histocompatibility Committee June 6-7, 2016 Board of Directors Meeting 1 How does this proposal support the OPTN Strategic Plan? Primary Goal: Promote living donor and transplant

  1. Adding HLA DQA1 Unacceptable Antigen Equivalences Table Histocompatibility Committee June 6-7, 2016 Board of Directors Meeting 1

  2. How does this proposal support the OPTN Strategic Plan?  Primary Goal: Promote living donor and transplant recipient safety:  Prevent matching candidates with incompatible donors and help avoid acute rejection, prolonged cold ischemia time, or organ discard  Improve waitlisted patient, living donor, and transplant recipient outcomes:  Reduces risk of rejection associated with HLA DQA1 mismatched donors  Promote the efficient management of the OPTN:  Aligns entry of DQA1 unacceptables with that for all other HLA loci for consistency in UNet SM 2

  3. What problem will the proposal solve? Selecting broad value as unacceptable Doesn’t exclude donors with a subtype value, and candidates can match with donors if subtypes aren’t selected 3

  4. How does the proposal solve the problem?  Creates an additional HLA Equivalence table for DQA1  Allows UNOS IT to Selecting broad value accurately program as unacceptable DQA1 unacceptable antigens Will exclude donors with a subtype value. 4

  5. Was this proposal changed in response to public comment?  No substantive changes made post-public comment  Stylistic changes for language consistency and ease of reading  Non-discussion agenda at regional meetings  0 No votes  ASHI, AST, & ASTS expressed support for the proposal as written 5

  6. How will members implement this proposal?  Members should familiarize themselves with changes in unacceptable antigen entry upon programming  Histocompatibility labs may need to work with third party vendors to update reporting systems  No new compliance requirements 6

  7. How will the OPTN implement this proposal?  This policy will be programmed by UNOS IT 7

  8. Overall Project Impact Product Policy Impacted Histocompatibility Labs Populations: Transplant Coordinators Total IT Implementation 1,125/12,820 Hours Total Overall 1,380/19,560 Implementation and Maintenance Hours 8

  9. Resolution 11 (page 47)  RESOLVED, that changes to Policy 4.10 (Reference Tables of HLA Antigen Values and Split Equivalences), as set forth below, are hereby approved, effective pending programming and notice to OPTN members. 9
