  5. “School Within a School” ∙ College & career ∙ CDE grant-funded Size: 280 students ∙ Family environment per academy (70 per ∙ Classes blocked together, close proximity grade level) Structure: students cohort and take 3-4 classes together daily. CHS By the Numbers 1960 - Clairemont High Established 2015 - Transformation to CHS Wall-to-Wall Academy Pathways 920 students grades 9-12 enrolled this year at CHS 4 Academies 19 Career-technical courses offered 120 Interns (12th graders) 193 Mentees (11th Graders) for 2018-19 Goal: Minimum 100 Mentors for 2:1 match ups

  7. About Our Students Get to know our demographics at CHS...these are the types of students you are mentoring. Ethnic Breakdown: 55% Latino | 40% Caucasian |>5% Other ● Socio-Economic: 60% on Free & Reduced Lunch Program ● Special Needs: 20% have IEP or SpEd identified ● ● 4 Year University Enrollment: 35% Community College Enrollment: 60% ●

Commitment You should have no problem committing to mentor if you can: ❑ easily prioritize monthly Fri meeting without conflicts ❑ be on time to every single scheduled meeting ❑ follow through and go above and beyond the required meetings ❑ have dedication and patience to help your student open up ❑ be available to communicate via e-mail ❑ see the program through to completion

Commitment: You should think twice before committing to mentor if you: ❑ are in the process of switching careers ❑ foresee a major life change that will restrict your time (wedding, baby, lengthy vacation, etc.) ❑ plan to move out of San Diego in the next 9 months ❑ need to travel unexpectedly for long periods of time ❑ have a schedule that’s very unpredictable and/or inflexible ❑ already see a conflict with two or more of the meetings

Mentor Program Overview * Group mentoring: 2 to 4 students for each mentor * Meetings once per month at 9am or 10:30 am * Focus on communication, goals, future, career-exploration & training * Group dynamic at meetings/ mentor as facilitator * Meetings are 60 minutes with a 15 minute intro * Job Shadows

  19. Processing & Paperwork : * CHS Mentor Contract * School Volunteer Application * TB Risk Assessment Questionnaire Completion Form or Completed SDUSD Volunteer TB Test Card * Live Scan: Request to Conduct Volunteer Screening Form * TB Test - copy of test within the last 4 years

TB (Tuberculosis) Test OPTION 1: *Turn in proof of Clear TB Test within last 4 years signed by a Physician OPTION 2: *Fill out TB Risk Assessment Questionnaire - if you answer “no” to each question - get it signed by a Medical Professional and submit! No test needed! *Our School Site Nurse, Sasha Lopez-Nusser, an RN, is available to sign these for you at the following times: 7:20 - 2 :20 pm on week days by appointment: (858) 273-0201 | alopez-nusser@sandi.net OPTION 3: *If you answered yes to any of the questionnaire questions, you must get a TB Skin test at your healthcare provider’s office, and use the SDUSD Volunteer TB Test Card

LiveScan Screening Process To complete through the SDUSD Police Services LiveScan Department : * Cleared prior to first mentor-student meeting * (If LiveScanned by SDUSD in last year, or employee, info should be on file.) * SDUSD Police Services Office in University Heights, 4100 Normal St. * Mon – Fri, HRS: 9:00 am – 3:30 pm * Take proper form of ID and Request to Conduct Volunteer Screening Form. To properly fill out this form: • Please check “CATEGORY D – Volunteer Fingerprint” at the top of the form. • For “REQUESTING SCHOOL” please write in “Clairemont High” • For “VOL. COORDINATOR,” please write in “Liz Rush.” • For “Volunteer Activity” please Check “Other” and write in “Mentor” * Say from “Mentor Program at Clairemont High”
