Tier 2 service for Adults and Families Adult Pathway Family Pathway Tier 4- Surgery- Tier 3- Specialist support- Psychological and dietetic support. Tier-2 Targeted programme adults BMI 28+ delivered in the community Tier 1- Universal Services- Open Access -The Gr@nd supports Adults with a Body Mass index of 28+ who are motivated and ready to make lifestyle changes - The Gr@nd support families with a child in who’s BMI is in the 91 st centile who are motivated and ready to make lifestyle changes
What is the Gr@nd doing • Funded by KCC Public Health until March 2015 • Review started Dec 2013 • Service Level Agreement- Sets out • Self Refer, GP referral, School Nurse Referral • NICE guidance -2006: Prevention, identification, assessment and management of overweight and obesity, -2007- Behaviour Change: The principles for effective intervention -2012: Preventing type 2 diabetes: risk identification and interventions for people at high risk -2013 Management of overweight and obesity in adults and children
Adult: 10-12 week programme • 3 courses per quarter at varied locations dependent on need including: Vigo, Cyclopark, Ekta Ghar, Istead Rise, Cascades, Cygnets NRG Gym. • People set their own Goals Specific Measurable Achievable Realistic Timely • Physical activity practical tasters • Nutrition • Follow up after the course finishes
Julies Story Julie joined the healthy weight course at NRG gym in October 2013 The course has given her that extra motivation to take a better approach to her eating habits and get more active. She is looking to join a gym in the new year hopefully if finances improve. Over 10 weeks she lost 10kg in weight and her waist measurement by 14.5cm.

Run a mile with a smile: Jan 2014 • Adult and family training programme in partnership with Revitalise Fitness • Support people who have never run before to complete the sport relief mile • 25 attending the adult session • 30 people attending the family session • 2 Ebbsfleet united players are visiting the group • The group have been invited to train at Ebbsfleet United
Fit to Football • KCC inequalities grant • March 2014-March 2015 • Engage 30 men • Work with Ebbsfleet United Football team.
