UNIVERSAL FAMILY CONNECTION, INC. PSYCHOSOCIAL SUPPORT SERVICES IMPROVEMENT PRESENTATION REASSESSMENT SCHEDULING TOOL A review of client files indicated that 11 of 53 clients from March 1, 2016 – February 28, 2017 were reassessed 30 to 45 days after actual date for reassessment Problems resultant from late reassessments included: • Out of date eligibility documents • Wrong client addresses • Staff unaware of new barriers to care

PSYCHOSOCIAL SUPPORT SERVICES IMPROVEMENT PRESENTATION REASSESSMENT SCHEDULING TOOL Two options for improvement of timely completion of reassessments: • Use of CAREWARE • Monthly Excel Printout of Client Reassessments' Dates Due that Month Improvement Cycle September, October and November 10 out of 12 clients scheduled for reassessment received assessment within 10 days of reassessment date
