Overview of Data Collection BEFORE the Screening Day Field Coordinator ADHS OOH sends school Screening T eam packs recruits school for Consent/Questionnaire and checks equipment, survey sample Packets supplies and forms for screening day County Contractor Screening T eam County Contractor and reviews survey schedules screening Screening T eam remind, protocols & logistics to day and assigns a promote and confirm prepare for screening Screening T eam consents/questionnaires day Health and Wellness for all Arizonans 14
Survey Events & Processes BEFORE the Screening Day 1. Field Coordinator recruits a school & request enrollment information 2. County Contractor & a Screening Team are assigned the school 3. County Contractor schedules a screening day with the school 4. ADHS OOH mails to school nurse/contact confirmation letter, teacher letters & consent packets for selected classes 5. Ask school nurse to encourage responses for consent & questionnaire and collect forms from selected classes 6. Call/email school nurse/contact one week before screening day to promote/remind/confirm 7. Screening Team packs supplies/forms and reviews screening protocols 15
Overview of Data Collection ON the Screening Day Check in at school For each child: Give “Referral” & office • Check consent “Findings” forms to • Conduct oral screenings school nurse/contact • Measure height/weight • Record Data/fill forms Meet school nurse or • Give goodie bag school contact Organize & group completed survey forms. Bundle forms by class and retain. Set up screening stations & wait area For each class: • Check forms • Count kids & consents Clean up. Survey Events & Processes ON the Screening Day 1. Arrive at school at least half hour before start of screenings 2. Check in at the School Office, ask for the school nurse or contact, and get the completed consents/questionnaires 3. Set up screening area (oral screening station, height & weight screening station, and waiting area) – aim for safety, privacy and efficiency 4. Discuss a game plan for the screening day schedule, calling students from classes, lunch break, etc. 5. Conduct oral, height & weight screenings (function as a Team to check consent, count students, collect/record data & sort forms) 6. Complete all required forms, group forms & bundle by class 17

Survey Processes AFTER the Screening Day • Use the HSHB Survey Form Submission Checklist to review, organize, and bundle all completed data forms for each school • Mail the completed data forms to ADHS Office of Oral Health within 1 week of screening day • Mail all completed data forms from each school to ADHS Office of Oral Health in 1 package (tape envelopes together) or in 1 box. 19
