Home from Hospital Service Developed in partnership with Bolton NHS Foundation Trust Hospital, to help older people make a safe discharge from hospital and to prevent readmission WHY IS THE SERVICE NEEDED? - Rising hospital admissions - Two thirds of people admitted to hospital are over 65 years - Older people stay in longer once admitted - 80% of delayed discharges are in the over 70’s - Many readmissions are avoidable - Re-admissions in 30 days cost NHS £2.2 billion From April 2017- March 2018 - 600 Clients were supported - 3255 Support Visits were made - 5.3% readmission of clients (against 10% average Bolton readmission rate in over 65’s)

About the Team Our office is at the Royal Bolton Hospital (Between C and D block) There are eight members of the team covering the Bolton area between 9am -9pm, 7 days a week Staff share a wealth of experience with backgrounds in e.g. Nursing, Primary Care management, and Community Care management.

Home from Hospital Referral Criteria - 65+ years of age - Living alone ( OR a carer for someone ) - A Bolton Resident - Recent hospital stay or visit to A&E Who can refer? - Self, friends or family - Any hospital or community worker (Physios, Occupational Therapists, Social workers, G.Ps, Nurses, Early Intervention Team, Home First etc.

  8. Home from Hospital Service Mr G is a 76 year old bachelor who lives alone in a 3 bedroomed house; he sees his elderly sister occasionally. He admits to being socially isolated and having feelings of loneliness. Until recently he drove his car locally and was able to do most daily tasks. He suffers from Parkinson’s disease and osteoporosis which has led to a quite marked curvature of his spine. In November 2018 he had a fall at home and following a short hospital stay he was discharged home with a Zimmer frame and some community therapy. A referral was made to the HfH team and an assessment was carried out. It was apparent that he had some considerable unmet needs. How we supported Mr G: - Attendance Allowance Application Needless to say Mr G has - Befriending Visits a safer and brighter future - Community Visits to Dominoes and Scrabble to look forward to. - Wiltshire Farm Foods - Care Line - Ring & Ride
