The CompNet resistance module: Current status Klaus Jansen Cohort Manager Competence Network for HIV/AIDS
Aims of CompNet resistance database Completing nationwide CompNet cohort with retrospective and prospective resistance data of patients Providing a sufficient basis for epidemiological and virological research on the CompNet-cohort Providing data to supplement national and international collaborations
Topics of scientific interest Prevalence of resistant HIV in population of CompNet-cohort Differences in therapy outcomes in patients having resistant HIV or in patients having not Differences in therapy outcomes in patients having a previuos resistance testing and harmonised therapy and in patients having no testing Analyses of impact of resistances on TCE Identification of new resistance patterns of new therapeutic regimens Impact of differing variants of HIV on clinical outcome of treated patients .....
CompNet-cohort Composition Basis-Module: 25 HIV-centres 10 clinics 15 special. private practitioners Resistence-Module: 9 Labs Children-Cohort: 8 centres Pregnancy-Cohort: 10 centres Neurology-Module: 1 centre Central Office CompNet CCT Cologne
CompNet-cohort Prospective und retrospective documentation of 283 soziodemographic and clinical items as well as collection of biological samples (using SOPs) Central characteristics of the cohort: Number of patients: ~ 8,200 Person years: ~ 43.500 in treated patients Proportion of women: 14.9% Risk of infection: 62.4% MSM, 18.3% Hetero, 7.5% IVDU, 4.8% HPL Therapy status: 8.9% ART-naive, 7.9% initially treated Mean duration since positive HIV-diagnosis: 10.3 years
Current status IT-tool for entry, storage and analyses: SmartGene-software Implemented targets: PR, RT, IN, gp41 Implementation of pilot centre Erlangen in 10/08 Implementation and software-trainings in 6 labs by now Completion envisaged within next weeks First step: compiling of retrospective data set manually till complete implementation of software Partly, greater efforts while identifying retrospective sequences due to changes in lab-documentation systems etc.
Current status Only inclusion of data of patients having international IC Only existing sequences are recorded, no new testing Current coverage of CompNet-patients by 9 labs: ~ 76% Current N of eligible patients: 1,406 Current N of documented sequences: 814 Expected N of eligible patients: ~ 5,500
Data and Networking Sharing of data set with researchers from inside and outside of CompNet appreciated Requests possible for virologic data as well as in combination with clinical and sociodemographic data Short proposal needed for conducting scientific projects Proposal evaluated by scientific boards and SC of CompNet After evaluation of proposal, data could be provided in different formats
Data and Networking Sharing of data with scientific networks highly appreciated: - national level: AREVIR, RESINA, Seroconverter-study - international level: CHAIN, EHR, EuResist… Important: analysing and avoiding overlaps with other German patient populations only slight overlaps expected CompNet population can supplement collaborations substiantially
Next steps Implementation of all labs Completion of international IC Completion of retrospective resistance data Ongoing documentation of prospective resistance data Compiling complete dataset after completion of retrospective resistance data Clarifying overlaps with other existing German populations Distributing data to researchers and research collaborations
Thanks To all collaborating laboratories and their staff To Virco for good collaboration To Hauke Walter And you for your attention!
