PLAN THE WAY FORWARD. • Stakeholders involvement. – Very important in decision making. – Can be time consuming when it comes to getting everyone together. – Can delay decision making. • Community presence. – Enables the person to have an identity in their community. – Leads to positive risk taking. – More relaxed approach from supporters. – Could be a good way for the person to build on their skills. • Person Centred Plan. – Sometimes people can plan according to what they feel is the right thing to do. • Advocacy. – Good impartial review of whether the service delivered is Individualised. – Questions the service provided which enables us to assess whether it works for the person. – Good relationship with advocates enables informed decision e.g. reducing support. 4

REVIEW AND CELEBRATE. • Regular team meetings – Focusing on positives regardless of how small they are. – Supportive of one another. – Looking at where the service was and where it is now. – Individualised support services can cause burn out. – The success is what keeps staff motivated and passionate. – Important for staff to bond. • Service review. – Continuously assessing the service being provided. – Budget. – Cost reduction. – Reducing support. • Involve the person…let them decide! • Celebrate the good things before moving on to the next challenge. 8
