Background The Denver Department of Human Services’ (DDHS) Family and Adult Assistance Division (FAAD) is responsible to administer eligibility for Colorado Works pursuant to Colorado Revised Statutes (CRS) at section 26-2-106, CRS 26-2-709, CRS, and Colorado Code of Regulations (CCR), 9-CCR-2503-6. DDHS is moving to a Tiered case management model for work eligible individuals (participants) in the TANF/Colorado Works program. Those Tiers appear in Section C of the Scope of Work/Services in the RFP.
Job Readiness Tiers Tier one – Job Ready. DHS case management with OED/ResCare One-Stop services. Participants have the required marketable vocational skills, commitment, and experience to gain and maintain entry level employment. Tier two – Short-Term Barrier Resolution. DHS case management with community-based contract services. Participants have at least some of the marketable vocational skills, commitment and/or work experience to gain or maintain employment. Tier three – Long-term Barrier Resolution. Community-based case management and services. Participants have time-limited barriers to employment as documented by a qualified professional. Tier four – Employment Alternatives. Community-based case management and services. Participants have permanent or long-term barriers to employment as documented by a qualified professional.
Vision DHS provides case management to participants categorized as Tier 1 and 2 with Office of Economic Development (OED) providing services to Tier 1. DHS partner with community-based contractors with expertise in working with individuals with barriers to case manage Tier 3 and Tier 4 participants. DHS responsible for initial assessment and referring participants, based on their job readiness levels, to appropriate service providers. Collectively, DHS, OED, and DHS contractors responsible for meeting Work Participation Rate.
Vision Participants will undergo a short assessment at DDHS after eligibility determination and will be subsequently referred to contracted vendors based on their level of job readiness (criteria in each Tier). Contracted vendors to do further assessment(s). Contracted vendors expected to fully utilize the workforce functionality in the Colorado Benefits Management System (CBMS), including documenting assessment results, work activities, supportive services, monitoring and reporting of work participation hours, which also includes employment entry and follow-up.

Ensure All ATTACHMENTS are completed and included in RFP PACKET. TERM OF SERVICES by selected proposers are to commence on June 1, 2017 through June 30, 2018. Contracts amounts and terms are contingent upon funding availability and contractor performance.
SELECTION OF CONTRACTOR(S): COMMITTEE- An evaluation committee will evaluate all proposals received prior to the proposal deadline for completeness and the proposer’s responsiveness to outlined specifications. GENERAL EVALUATION CRITERIA- Evaluations based on a minimum average score of 70 points out of a possible 100 points to be responsive to this RFP. DISQUALIFICATION- Failure to provide all requested information may result in disqualification of the proposal. INTERVIEWS- An interview may occur to gain a better understanding of the work to be performed. Proposers selected to be interviewed will be contacted and scheduled, in no particular order, to meet with the evaluation committee or a representative of DDHS at the City’s sole discretion. RECOMMENDATION- The evaluation committee will provide recommendations to the DDHS’ Executive Director of the proposer(s) whose proposal(s) is most responsive to the specifications as stated in this RFP. DECISION -Upon the decision of the Executive Director, the contract will be prepared and submitted for signature through the City’s contract approval signature process.
