GP, pharmacy and community dental services). • Population around 505,000 and increasing

History of the Francis Report • Mid Staffordshire NHS Foundation Trust, failures in care during the period of 2005 - 2009 • Feb 2010 – Independent Inquiry into events at the Trust; first report, Robert Francis QC • 18 recommendations • Identified the need for wider learning • Call for Public Inquiry • Feb 2013 - Public Inquiry into the wider learning, the role of multiple agencies. • 290 recommendations

Locally – The independent Inquiry (1 st report) • 2010 report recommendations led to all Trusts undertaking review locally • 2010 we undertook gap analysis, and produced an action plan. • We invited several patient representatives to be involved. • Board report - presented with patient representatives • Presented back to the public and our Trust members at patient safety event in Feb 2011

Locally – The Public Inquiry 2 nd report • 2013 – Published report with 290 recommendations! • Not all relevant to healthcare providers • Identified relevant recommendations for us • Reported the analysis to Trust Board • Need your input to develop specific actions • Tonight ! “Talking Francis” We would like to hear your ideas and solutions………….
