Network 3500 Covered Services Network 3500 Plan* The Spirit Network plan pays for covered dental Network 3500 Covered Services expenses for network, and non-network providers*, based on the contracted fee amount negotiated after the $100 lifetime deductible has been satisfied on Preventive, Basic and Major Services. These Percentages are: • 100% for Preventive services, 50% for Basic, 25% for Major in year one, with a $1,200 annual maximum, and 10% coverage for Ortho Services In year two, Basic Services increase to 65%, 50% • for Major with a $2,500 annual maximum, and 25% coverage for Ortho services • In year three, Basic services increase to 80%, with a $3,500 annual maximum, and Ortho services increase to 50% *Non-network provider reimbursement based on what PPO contracted fees would be if using a network provider.
