About us….. • Foundations of a local maternity system across Somerset • 2 consultant led maternity services – total 4700 births • 1 stand alone MLU, 1 alongside MLU, 1 planned MLU for 2017 • Home birth and vulnerable women’s teams • Different models of care traditional & integrated • Partnership working already in progress – Plus South West Maternity Clinical Network and Obstetric Network • Joint working on MDT perinatal mental health pathway • A Equip Supervision Pilot site 3 Aiming for the Best in the South West

Going digital … • The connected mother and family • A digital information pathway choice app – supporting women to make the right care choices • My baby, my pathway – accessible shared records • Effective, real-time communication with a named midwife/team • Self care programmes with information, signposting and access to support (baby movement, smoking, weight, wellbeing etc.) • Our deeply rural area require a mobile, flexible workforce that is digitally connected, with user-friendly, app based technology 5 Aiming for the Best in the South West
Improved postnatal and perinatal mental health care • Improved access and continuity of care • Develop existing vulnerable women’s teams strengthening links across organisational and professional boundaries • Develop a new postnatal practitioner role to support the most vulnerable working across maternity and health visiting boundaries linked to Nurse Associate Programme • Together with public health fully implement an MDT perinatal mental health pathway 6 Aiming for the Best in the South West
