Access Community Health Network PSS Quality Improvement Presentation: Support Group • Rationale: To increase the number of clients at support group at Madison Family Health Center • Baseline data 2016-2017 grant year: One client attended support group at Madison • Strategies for improvement: • Change from bi-weekly meeting to quarterly meeting • Cancelled support group at Grand Boulevard Health and Specialty Health Center to allow clients to attend at Madison • Flyers for promotion • Survey conducted to gather patient interest in support group and assist in choosing topics • Different topics for each support group session
