  8. Active Forth What conditions do we cover? See referral form All conditions must be stable! If referring to classes only, we still need referral form What do we need from you on the form? As much information as possible Any current treatment, medication Any appropriate medical history

Within 3 months, can restart membership After 3 months, need to re-refer Members can freeze their membership for up to 12 months

  13. Active Minds Active Minds – What are we doing? Work with health professionals within the NHS and community wide to offer a referral pathway for those with a mental health condition/s to take up physical activity. Where appropriate integrate clients into site wide activities including the gym, Step Forth, swimming and sports. What will it cost? £20 per month, £2 for classes and groups Free trials

  14. Step Forth FCT’s Walking Programme Started in 2003 as a Walking for Health programme Now delivering a range of walking activities Nordic Walking Buggy Walks Otago Class Helix Walks Walks across Falkirk

  15. Step Forth All walks are free Nordic Walking – participants must do initial training course at a cost of £20, walks are free thereafter Class costs £2 per session, or free as part of Active Forth membership
