  1. Hello llo, I I am the Community Lin ink Advis isor, part of the new Social Prescribing service available in GP Surgery’s across Staffordshire . My objectives for today: • Introduce myself and the Charity I work for • Let you know what Surgery’s we cover • Explain our hope • Let you know who we work with • Share the referral pathway • Describe a typical patient journey • Provide time for questions at the end

  2. What is Social Prescribing? • The process where a person is ultimately connected into the community. • Where an individual will come for an appointment and meet a Community Link Advisor to discuss what matters to them. • Co produce a plan of action to work through. • Find out about different groups, services are out there in the community easily accessed.

  3. What is the role of a community Link Advisor? • To build rapport with the individual. • Have a good conversations and listen. • To help the individual to consider positive steps forward towards their goals. • Have excellent knowledge of the local and wider community. • Have connections with community groups and services. • To have excellent communication skills.

  4. Link Advisors and Surgery’s Stafford Town PCN - Wolverhampton Road Surgery, SHAW and Holmcroft Surgery. Stone and Eccleshall PCN – Mansion House, Cumberland House and Crown Surgery. South Staffs PCN – Brewood Medical Practice, Penkridge Medical Practice and Rising Brook Surgery. Seisdon PCN - Bilbrook Medical Practice, Codsall - Russell House, Perton – Lakeside Medical Practice and Tamar Medical Practice, Featherstone Family and Health Centre, Wombourne – Dale Surgery and Gravel Hill Surgery, Claverley Surgery, Kinver – Moss Grove Surgery.

Who we work with? • Anyone you feel is struggling with social, economic or environmental issues that go beyond the medical. • Anyone you want to access a more holistic approach to their health & wellbeing. • Anyone not benefitting from current treatment plans who you think might have other, underlying issues. • Anyone with Long Term Conditions who you feel would benefit from additional community based support. • Anyone with low level mental health issues you feel are precipitated by loneliness, isolation, grief, etc. We can work with individuals to design community based solutions & are always happy to discuss referrals.

  6. How to make a referral • Firstly the person is required to be a patient of one of the Surgery’s we are based in . • Ask the individual to call or visit their surgery to make an appointment with the Community Link advisor.

  7. Typical Patient Journey • Referral – Appointment made • Initial Review Appointment this will take up to 45 minutes (individually tailored): • Additional Details • Safeguarding • Personal Plan and Goals • Progress review appointment and/or contact (up to 4 interventions before reassessment - individually tailored): • Review goals • Consider progress and obstacles • Support sustainable connections and positive change We can meet people in the surgery for 1:1 appointments as well as in the community and do home visits and individuals can call to discuss progress throughout their time with us. Working with the public and 3 rd sector to fill gaps as and when identified.
