  2. 4.b.i Promote Tobacco Cessation • 4 PPSs working with DOHMH Bureau of Tobacco Control: Advocate Community Partners , Nassau Queens, Lutheran, New York Presbyterian • In January, we convened the four PPSs that chose project 4bi) to discuss – Potential technical assistance in the clinical and non-clinical setting, – A strategy to collectively contribute to a larger citywide media plan, and – Potential improvements to the tobacco cessation benefit offered by Medicaid and commercial plans • The next meeting will take place in April with all PPSs to continue these discussions

  3. 4.c.ii. Increase Early Access and retention in HIV Care • 7 PPSs working with DOHMH HIV related programs: Bronx- Lebanon, HHC, Lutheran, Maimonides, Mt. Sinai, NY Hospital of Queens, St. Barnabas • Most Domain 4cii work has taken place through an HIV Joint Planning Group, which includes all NYC PPSs who have proposed Domain 4cii Projects. • Potential Domain 4cii Projects that will include collaboration with DOHMH include – Viral Load Suppression Project (2-3 PPSs) – PrEP for High Risk Negatives (6 PPSs) – Anti-Retroviral Treatment and Access to Services (ARTAS) intervention (2 PPSs) – Peer Specialist Health Navigation Services (3-4 PPSs) – Social Marketing, Consistent Messaging, and Appointment Procedures (4 PPSs) – 3-Tiered Cultural Competency Project (4-6 PPSs) – Promote Delivery of HIV/ STD Partner Services (1 PPS)

  4. 3.d.ii Expansion of home based self-management program 3.d.iii Evidence- Based Medicine guidelines for asthma • PPSs interested in working with DOHMH on Asthma Projects: Bronx Lebanon, HHC, Maimonides, NY Hospital Queens, St. Barnabas • In the PPSs that have connected with DOHMH, all are interested in coordinated borough wide efforts so that patients receive standard messaging throughout Borough • DOHMH is offering support in the following areas of collaboration. – environmental home assessment training and technical assistance (4 PPSs) – educational materials development, (2 PPSs) – electronic health records/IT systems referrals and coordination, (3 PPS) – school health and tobacco cessation (3PPSs)

  5. DOHMH Primary Care Information Project • 6 PPSs working with DOHMH PCIP: Bronx- Lebanon, HHC, Maimonides, Mount Sinai, New York Presbyterian, St. Barnabas • PCIP offers assistance on Domain 2 projects that are integrated with Domain 3 & 4 projects – Partner Reports: Analysis of provider networks and their status on EHR, Meaningful Use, and PCMH Recognition Status (“Partner Report”) Completed (3 PPSs): HHC, Bronx Lebanon, Mount Sinai – Advising several PPS Leads on strategies for EHR adoption (3 PPSs): HHC, Maimonides, St Barnabas

  11. 3.a.i Integration of Primary Care and Behavioral Health 4.a.iii Strengthen MH Substance Abuse Infrastructure • 3.a.i. All PPSs working on integration of Behavioral Health and Primary Care – DOHMH convened a NYC Behavioral Health PPS meeting 2/13/15 – Behavioral Health PPS leads will participate as Steering Group to the NYC Regional Planning Consortium led by DOHMH • 4.a.iii. 5 PPSs working together on strengthening mental health and substance abuse infrastructure – These PPSs have been meeting together to focus on school related preventive interventions and an collaborative care model adapted for an adolescent focus – DOHMH has been collaborating with these PPSs as a group in preparation of a possible implementation plan to partner them with Community Schools in their adoption of a school-based mental health model
