What is Boulder Connect? Community Case Management Boulder Connect is…. A community-wide, integrated case management and resource navigation platform. Built on top of Boulder’s integrated data warehouse using a secure government Salesforce cloud platform. Delivered as a software as a service Service Service option to community partners. Connects Boulder County Integrated Party Data Warehouse Kimber to other 1 Kimber shows up at the shelter and the appropriate services. community provider assesses for risk, reviews existing data and submits a referral to Housing Panel for assistance.

CORHIO Partnership Opportunities • Combine CORHIO expertise in data integration and data exchange with Boulder County’s innovative SODH technology approach. • Develop a data governance framework to support a Social Information Exchange concept. • Refine the Boulder Connect software product to allow for expansion to other communities. • Create an opportunity to widen the scope of the “care team” to better coordinate with community and social services case management staff and leverage community resources more directly.
