3 Meal Pattern Waiver App ppli lica cation tion Walk alk Thr hrou ough gh • CE Name - Enter the name of the contracting entity in the designated box • CE ID - Enter the five-digit CE ID number for the contracting entity in the designated box • Impacted Programs Select the program you are operating that needs the meal pattern flexibility. You may select multiple programs. • NSLP/SBP – SY 2020-2021 • SSO – Summer 2020 • SFSP – Summer 2020 • CACFP

Milk – most requested waiver during summer feeding due to manufacturers limiting the types of milk being produced or limitations set in stores
5 Meal Pattern Waiver App ppli lica cation tion Walk alk Thr hrou ough gh Examples of approvable submissions: • Produce vendor shorted order of carrot sticks; therefore CE will not have enough Red/Orange vegetables this week until the next order is received • CE operating CACFP cannot purchase enough vegetables, fruit, MMA, or grains due to grocery store purchasing limits set on specific food items
6 Meal Pattern Waiver App ppli lica cation tion Walk alk Thr hrou ough gh Waiver for disruptions to the availability of food products • Non-approvable reasons – Lack of packaging – Lack of storage – Lack of staffing Provide any attempts such as borrowing from another district or talking to another food service vendor
7 Meal Pattern Waiver App ppli lica cation tion Walk alk Thr hrou ough gh Start and end date are based on your ordering schedule. Waivers will not be approved for dates that exceed 2 weeks. Provide what meal pattern alternatives you are serving such as serving a different vegetable component or serving more enriched grain products if whole grain rich products are not being served.

TDA may contact the CE for information on how the waiver was utilized. Identify a contact responsible for responding to all TDA follow up required in relation to COVID-19 meal operations.
