What is the DAP-EPS? • The Diagnostic Assessment Program- Electronic Pathway Solution (DAP-EPS) is designed to provide shared health information to DAPs, healthcare providers and patients and their family and friends • DAP-EPS is a joint initiative between Cancer Care Ontario , Canadian Cancer Society and Canada Health Infoway • After completing a pilot project for Lung and Colorectal DAPs at Waterloo Wellington Regional Cancer Program and Regional Cancer Care Northwest, provincial roll-out is under way 3
How Does the DAP-EPS Work? The DAP-EPS consists of two websites: ! ! The DAP- EPS is: • an interactive tool • built on clinically validated disease pathways • designed to empower patients as they progress through their diagnosis journey 4
The DAP-EPS: Empowering Patients Your Health, Your Diagnosis, Your Journey ! Patients are encouraged to participate in the management of CONTROL! their health ! For the first time in Ontario, patients have direct access to the same diagnostic pathway and test results physicians use KNOWLEDGE! for diagnosis ! Patients have a comprehensive list with contact information FAMILIARITY! for all their doctors and family or friends involved in their care ! Patients know when their test results are available TIMELINESS! ! All appointments and information is current 5
