About Hospices • Hospices seek to improve the quality of life and wellbeing of adults and children with a life-limiting or terminal illness, helping them live as fully as they can for the precious time they have left • Hospices offer wide-ranging, personalised care which is provided by a range of different professional staff and volunteers. As well as taking care of people's physical needs, they also look after their emotional, spiritual and social needs. • Hospices also support carers, family members and close friends, both during a person's illness and during bereavement.

About Trinity Hospice • Founded in 1891 • Based on Clapham Common • Covers 750,000 people across Central and Southwest London • Supports patients with a range of illnesses, including cancer, heart and lung disease, motor neurone disease and dementia. • We receive less than a third of our funding from the NHS. • All our services are free of charge.

Hospices and BME communities • Evidence that people from Black, Asian and Minority Ethnic (BME) groups are less likely to access end of life care services • Barriers include: – Lower awareness of hospices among BME communities – Assumptions made by referrers e.g. families from BME communities want to deliver care – Less likely to talk about and plan for death due to cultural and religious influences e.g. God may be seen as the one to decide life and death • We are here today to raise awareness of our services and to understand how we can better meet the needs of all the people we serve

  7. 1. Community For patients who wish to be looked after at home. • Community Clinical Nurse Specialists • Consultants • Occupational therapists • Befriending volunteers* • Hospice at Home**

  8. 2. Outpatients (Day Hospice) Support at the hospice for patients living at home. • Physiotherapy • Group classes e.g. art, exercise • Complementary therapy • Nurse and Consultant Clinics • Transport available

  9. 3. Inpatient Provides symptom control, rehabilitation and end of life care at the hospice. • 28 beds • Individual ensuite rooms and bay wards • Visiting times open • Families welcome to stay

  10. 4. Patient and Family Support Emotional and practical support to patients and their families. • Counselling • Welfare benefits advice • Practical planning • Bereavement support • Available across all services

  11. Spiritual Care • Multi-faith Spiritual Care Lead and volunteers • Faith leaders regularly visit hospice • Our chefs cater to specific diets e.g. halal, kosher • Spiritual space under development for Autumn 2015 • We respect a patient’s personal wishes

  14. How to refer Referrals are accepted from: • Healthcare professionals (GP, Hospital Doctor, District Nurse) • Other providers of support such as social workers* • Self referrals from patients and those close to them* Please call 020 7787 1065 to speak to our Referral and Admission Nurse.
