  2. Supplement 160 This Supplement deals with • the instruction which RT Radiations are delivered and • the definition of the patient setup for treatment delivery. 2

  3. Delivery Workflow • A Treatment Planning System (TPS) generates a "treatment plan" by creating one or more RT Radiation Sets, referencing one or more RT Radiations (see Supplement 175). • These instances are sent to the Treatment Management System (TMS). • The TMS instructs the treatment device to deliver all or a subset of these instances • The instruction is realized using a UPS containing a RT Radiation Set Delivery Instruction as input information 3

Delivery Instruction The RT Radiation Set Delivery Instruction contains details about • the current fraction number • which RT Radiations are to be verified or delivered • where to continue a delivery that was interrupted in a prior treatment session • in which order the RT Radiations are delivered • which RT Radiations are omitted during this session. 4

Patient Setup • Before treatment, the patient has to be set up depending on the treatment location and the indication for treatment in order to guarantee safe and reproducible delivery results. • The setup parameters are defined during the treatment planning process. • Treatment therapists will utilize the patient setup parameters as an instruction for their setup procedure. 5

Patient Setup Parameters The RT Patient Treatment Setup IOD contains the following information: • The orientation of the patient with respect to the treatment device and gravity • The kind of setup device (e.g. treatment table or chair) • The setup method (e.g. at isocenter or by a defined distance) • The kind of shielding devices to be used • The method how the patient is aligned (e.g. infrared markers or surface scanners) • Whether certain procedures are to be considered during setup (e.g. patient is sedated or vital signs have to be monitored) 6
