BACKGROUND • Over 2,000 sheltered and unsheltered homeless at any point in time in Riverside County (2016) • Homeless/unstably housed individuals suffer from chronic health conditions and may be high utilizers of care – Most individuals qualify for/are enrolled in Medicaid Riverside County Department of Public and Social Services, 2016; Karaca et al., 2013

BACKGROUND • Permanent supportive housing (PSH): permanent housing with indefinite leasing or rental assistance paired with supportive services – Definitively associated with improved health – Promising findings regarding decreased medical and social services costs U.S. Department of Housing and Urban Development, 2017; Cassidy et al., 2016

  5. IEHP HOUSING INITIATIVE • IEHP Housing Initiative – Funds for PSH for IEHP Members across Riverside and San Bernardino Counties – Focus on vulnerable Members with high levels of health care use – Challenge: identification of eligible Members
