ESCAN – AN INTEGRATED SERVICE FOR CHILDREN WITH ADDITIONAL NEEDS • Commissioned by Ealing CGG and Ealing Council since 2006 • ESCAN services are provided by Ealing Hospital NHS Trust and Ealing Council and Ealing Council • Provides specialist child health, social care and special educational needs services • Central site, one front door, single management structure – in the heart of the borough • Parent feedback – very positive • Staff feedback – helps professionals to share learning and expertise
SAFE – AN INTEGRATED SERVICE PROVIDING EARLY HELP TO FAMILIES • SAFE – Supportive Action for Families in Ealing • Commissioned by Ealing Council since 2006 • Services provided by West London Mental Health NHS • Services provided by West London Mental Health NHS Trust (eg clinical psychology) and Ealing Council (eg family support and parenting ) • Provides help to families with the aim of preventing problems escalating • Provides tier two CAMHS help
0 TO 5 – AN INTEGRATED EARLY YEARS SERVICE • Plans in place to have a fully integrated health and early help service in place in 2015 delivering much of the healthy child programme and help to parents and families • To include the health visiting service, family workers and health professionals • To be commissioned by Local Authority • CCG to consider whether to jointly commission with the Local Authority • Service to have particular focus on attachment, nutrition and communication
SUPPORTING CHILDREN AND YOUNG PEOPLE WITH LONG TERM CONDITIONS TO LIVE AT HOME • Asthma nurse – manage care plans with parents and provides support and training to schools • Children’s specialist community nursing service � Home nursing service covers general nursing, asthma and haemoglobinopathy. � Includes continuing care services. � Diabetes BPT consultant led pathway
HOSPITAL SHORT STAY ADMISSIONS & A&E/UCC ATTENDANCE • Paediatric Assessment Unit � Children observed over a 12 hour window with a view to supported discharge home. • Frequent attendance to A&E/UCC Frequent attendance to A&E/UCC � Parents/carers are contacted (telephoned) by staff from children community nursing services, to provide advice, support to parent & carers in the management of their child’s condition
CONNECTING CARE FOR CHILDREN • Improve GPs expertise in managing paediatric cases in primary care. • Reduce dependency on children’s out-patient departments • Joint Outreach Clinics with paediatrician and hubs of 4-6 practices. • MDG with paediatricians and GPs. • Mapping exercise underway to implement this provision in Ealing.
