We have discussed and I am fully aware of: Safe and unsafe behaviour The need to report hazards and unsafe acts committed by others The need to look after the safety of myself and others That nothing is so important it is worth putting ourselves at risk for The company does not expect us to take risks If I am unsure of what to do at any time I should ask . I should not attempt to carry out any job or operate any machinery I have not been trained for. I understand that if I am not clear on any of the above points I have the right to say so and have them further explained.
