ELIGIBILITY
Eligibility Requirements 1. Program must have an approved Swim Lesson Provider Network application, or 2020 Learn to Swim Grant application, on record in the SurveyMonkey Apply platform. Program must adhere to the network requirements 2. as listed below: Instructors and lifeguards hold current CPR and First Aid • certifications Maximum 6:1 student-to-instructor ratio • Minimum four (4) hours in-water instruction per child • Program maintains current liability insurance with the • following minimum limits: Each Occurrence: $1,000,000 o General Aggregate: $3,000,000 o Program maintains formal, practiced Emergency Action • Plans for each location or program site Program facilities are regulated by the Department of • Health or similar governing agency
Eligibility Requirements (continued) Program must have and maintain throughout the grant 3. calendar-year, a current insurance certificate naming the USA Swimming Foundation as an additional insured, or be self-insured with a signed Indemnification Statement and Picture proof of insurance on record. Program must have a valid Federal Tax ID 4. Number/Employer Identification Number (EIN) Program must include the USA Swimming Foundation logo 5. directing to usaswimming.org/foundation on their website.
GENERAL INFORMATION
2020 Grant General Information USA Swimming Foundation funds are intended to expand current • programming to reach under-served and under-represented populations and communities that would not otherwise have the opportunity to learn to swim. . Applications will be categorized/grouped based on the total number of • children receiving swim lessons from the applying organization (total among all locations) on an annual basis. Funding is awarded based on the number of additional children the • organization intends to serve with USA Swimming Foundation Learn to Swim Grant dollars. Funding can be applied towards: • Free or reduced-cost youth swim lessons (i.e. scholarships) o Transportation to/from the pool o USA Swimming Foundation funds are not intended to provide budget • relief, support facility overhead, construction or renovation, aid in the purchase of material goods (including equipment, apparel, etc.), or be used for advertising, promotional, or marketing purposes. Grant applications are due no later than: • December 15 th at 8 pm EST / 5pm PST
PROCESS
Application Process All applications will be submitted through the online • platform, SurveyMonkey Apply There are three (3) tasks to complete to fully-submit • the application: Grant Application o Budget Form o W9 Form Upload o Link to apply: • https://usaswimming.smapply.io/prog/usa_swimming_fou ndation_network_and_grant_application/
If you have not yet created an account with SurveyMonkey Apply you will first need to Register and complete the Swim Lesson Provider Network Application, otherwise click “Log in”
When you log in you will see your previously submitted applications. Click on “Programs” to access the current grant application
Click on “Apply” to open the application process
Click on each of the three tasks shown and complete
Each of the three tasks has a “Mark as Complete” button that MUST be clicked to finalize the task
Once all three (3) tasks are completed, you MUST click “Submit” to review and submit your full application
Application is not fully submitted until you click “Submit” here
Once fully submitted you will see this pop-up as a confirmation, and an email will be sent to the primary administrator
Applications will show an updated status in real-time as they move through the process
