  5. What’s New? A new Non-Medicare medical plan is being added for 2020. For those of you who are non- Medicare eligible, we will be offering the Anthem Blue Cross High-Deductible Health Plan (HDHP). This plan has a $1500 deductible for an individual, $3000 for family (2 or more) and then pays 90%. What Actions Can You Take? ▪ Change to a different medical plan - See if the Anthem HDHP is an option for you ▪ Change to a different dental plan (California only) ▪ Suspend your coverage (opt out) ▪ Enroll in a plan if you had previously suspended coverage ▪ Enroll or cancel eligible family members ▪ Enroll or add dependents in ARAG Legal If no changes are made during Open Enrollment, you will remain in the same plans. 5 LLNL-PRES-795489

Anthem Blue Cross HDHP Offers both In-network and Out-of-network benefits ▪ In-network: Deductible: $1,500 individual; $3,000 family (2 or more) – Coinsurance: Plan pays 90% after deductible is met – Out-of-Pocket Maximum: $5,000 individual; $10,000 family – ▪ Out-of-Network: Deductible: $3,000 individual; $6,000 family (2 or more) – Coinsurance: Plan pays 70% after deductible is met – Out-of-Pocket Maximum: $10,000 individual; $20,000 family – ▪ HSA Compatible LLNL-PRES-795489

  15. HSA Compatible An HSA is a tax-advantaged account which can be used to save money for eligible medical expenses. Some advantages of an HSA are: ― Contributions to an HSA are tax deductible ― Earnings grow tax free ― Distributions used for eligible medical expenses are tax free ― Unused HSA balances rollover from year to year The 2020 IRS HSA maximum contribution limits are: ― $3,550 for a single plan and $7,100 for a family plan ― Additional $1,000 if you are age 55 or older You may want to ask a tax advisor if an HSA plan is right for you.   16. Medicare Becoming Medicare eligible ▪ You can be eligible for Medicare as follows: - Normally at age 65 - Before age 65 if disabled and receiving Social Security disability benefits ▪ If eligible due to a disability, contact Empyrean 15 LLNL-PRES-795489

  17. Medicare ▪ If Turning age 65 - Are you eligible for Medicare? Contact Social Security Administration (SSA) to find out – If you are currently receiving Social Security check, you will be automatically enrolled in Part A & B and will be sent an ID card – If you are not eligible for Medicare, SSA will provide you with a letter that you will need to send to Empyrean as proof ▪ If Due to Disability - You will be notified by Social Security Administration when you become eligible for SSDI benefits 16 LLNL-PRES-795489

Medicare ▪ If Turning age 65 - Three to four months prior to becoming 65, a letter will be sent to you from Empyrean – Option 1 : Enroll in Kaiser Senior Advantage through Empyrean – Option 2 : Enroll in other individual Medicare supplement plans through Via Benefits – Option 3 : Not eligible for Medicare, stay on Non-Medicare plan – Option 4 : Suspend LLNS coverage and enroll through spouse’s plan - Non-Medicare dependents will continue in the same plan with direct contributions Contact Empyrean at (844) 750-5567 to confirm your selection and finalize the event 17 LLNL-PRES-795489

Medicare - What’s New? ▪ New Medicare Card ‐ Have you received a new card and destroy your old Medicare card? ‐ Your new Medicare Number is a unique combination of numbers and letters – it no longer uses your Social Security Number Watch out for scams! Medicare - Coverage Options ▪ Kaiser Permanente Senior Advantage (KPSA) ‐ Election made through Empyrean (for California only) ‐ Group plan ‐ Monthly cost is $240.20 per Medicare eligible person Annual cost is $2,882.40 – $432.40 annual out-of-pocket expense if receiving 100% LLNS – contribution to HRA ‐ New enrollees can now enroll in KPSA online with Empyrean – no enrollment forms! ‐ Online submissions must be completed and submitted to Kaiser by December 31 st for January 1, 2019 effective date 19 LLNL-PRES-795489
