WGHST Working Group on Health System Transformation Meeting of December 11, 2019
The WGHST is a subcommittee of PSL’s Governance Committee tasked with assisting the PSL Board in establishing a governance relationship with each of the Brampton and Mississauga OHT’s. a) Represent the Board in meetings and discussions related to the health system transformation initiatives of the Ontario provincial government with the Mississauga and Brampton OHTs. b) Assist the Board in partnering with OHTs to deliver an integrated, full and coordinated continuum of care to the population of Mississauga and Brampton, in line with the vision of the Ontario provincial government. c) Promote effective communication with OHTs , the regional governments, the provincial government, public and other stakeholders. d) Ensure that the Board develops an effective working relationship with OHTs , the regional government and the provincial government.
The Working Group promotes open lines of communication to ensure positive working relationships with OHT Members, Regional representatives and Provincial representatives. • promote PSL Board’s participation in OHT formed sub-committees wherever possible to ensure a voice • consider all points of view prior to taking a position on a decision • promote adoption of the decisions of the OHTs within PSL to ensure alignment
Non-aligned Governance Representatives • observe each of the two OHT’s interim governance collaboratives • assist and contribute to the evolution and design of a longer-term governance model for the OHT • obtain representation on each of the OHTs if possible
The WGHST, through the PSL Board, promotes decisions and policies that balance the needs of all Members of the OHTs that ensure Teaming - promote cooperation, agreement, clear expectations, effective decision-making Transparency - timely and accurate disclosure Accountability - measurable results of decisions Fairness - equitable treatment of all Members Responsibility - member self-interest must align with the OHT and public interests Sustainability - efficient use of resources
The WGHST advocates for PSL’s desired outcomes, which include: • gaining representation on each of the two OHT Boards • a meaningful voice in the decisions made by the OHTs • a fair and predictable financial funding formula • retention of self governance • retention of intellectual property • the financial and legal ability to retain existing PSL staff and maintain our autonomy to grow the organization’s reach • maintain the existing high level of care currently provided to clients • the autonomy to add additional areas of service offerings
The PSL Board’s objectives must ultimately align with the desired outcomes of the OHT and Provincial Government. The Ministry’s vision • Full and coordinated continuum of care • 24/7 access to coordination and navigation • Improved performance on the Quadruple Aim : • better population health outcomes • better caregiver experience • better provider experience • better value • Single, clear accountability framework • Integrated funding envelope • Reinvest into front line care • Improved access to digital tools
The Ministry’s desired outcome is a health care system that is: • cost effective with high value for money • provides integrated care across the continuum, prenatal to end of life • ensures caregiver coordination among all providers • ensures local availability of all services • user friendly to navigate via telephone and internet 24/7 • promotes easy patient transition between providers • focused on avoidance of unnecessary emergency visits and hospitalization
Each OHT should ideally contain Members from (3 minimum) 1. primary care (including inter-professional primary care and physicians) 2. secondary care (e.g., in-patient and ambulatory medical and surgical services, including specialist services) 3. home care 4. community support services 5. mental health services 6. addition services 7. health promotion and disease prevention services 8. rehabilitation and complex care 9. palliative care (e.g., hospice) 10. residential care & short-term transitional care (e.g., in supportive housing, long-term care/retirement homes) 11. long-term care home placement 12. emergency health services 13. laboratory and diagnostic services 14. midwifery services, and 15. other social and community services and other services, as needed by the population.
An OHT’s principles are to 1. Support the health of the whole population 2. Create one seamless system 3. Provide access to holistic care , with a foundation in Primary Care 4. Empower patients and caregivers and deliver exceptional experience
The PSL Board forms partnerships with OHT Members Partner with OHT Members , and in particular OHT Members that provide similar services, to develop a common, workable plan to deliver the desired Provincial outcomes.
