Hold the Gains Projects Agents against Harm- Screening for VTE risk in Children – Niswonger Children’s Hospital – Children’s Administration - As one of last year Level IV recipients, this project has sustained its 100% improvement rate in screening the pediatric patient for risk of Venous Thromboembolism (VTE). Niswonger Children’s Hospital VTE Screening tool has been shared and used as a template for other children’s hospitals expressing an in terest in Solutions for Patient Safety network in the United States and Canada. Schreiber Beating the Clock: Discharged before 2pm – Johnson City Medical Center – Patient Throughput - The focus of this project was to implement a process resulting in earlier patient discharges, thereby decreasing the daily bottleneck that occurred in our flow through the Emergency Department and decreasing patient holds while waiting for a bed.
