  2. University of Chicago MS/PSS Quality Improvement Presentation: MS Documentation ▪ Need to improve the documentation process of mental health encounters for assessments, informed consent, and eligibility ▪ Taking a closer look at the results of the site visit, the team thought that the activities were being done, but that documentation did not always take place ▪ Baseline data from site visit: % of charts containing all criteria for: n % Initial Mental Health Assessment 25 4% Patient Rights and Responsibilities and Grievances 28 54% Confidentiality 28 57% Informed Consent for Mental Health Treatment 28 43% Eligibility Criteria 28 57%

  3. University of Chicago MS/PSS Quality Improvement Presentation: MS Documentation ▪ Piloted a redesign of the behavioral health intake process • Used social work interns to serve as Intake Specialists that follow up with referred clients, complete intake, and schedule an initial appointment with a behavioral health specialist ▪ We saw improvement in documentation across the board: Baseline Pilot Difference % of charts containing all criteria for: n % n % Initial Mental Health Assessment 25 4% 15 53% ↑ 49% Patient Rights and Responsibilities and Grievances 28 54% 15 100% ↑ 46% Confidentiality 28 57% 15 100% ↑ 43% Informed Consent for Mental Health Treatment 28 43% 15 100% ↑ 57% Eligibility Criteria 28 57% 15 60% ↑ 3%

  4. University of Chicago MS/PSS Quality Improvement Presentation: MS Documentation ▪ Successes • In addition to the significant improvements to documentation, we found that the new process also helped reduce the rate of no shows for initial behavioral health appointments. Since clients need to complete the intake and initial assessment before the first appointment with a behavioral health specialist, the new process helps filter out clients that are not yet ready to commit to behavioral health services. ▪ Challenges: • New model is dependent upon having the additional resources of social work interns ▪ Next • Keep working to improve documentation rates, especially for the initial mental health assessment and eligibility • Discuss sustainability and contingency planning for the new intake process
