ENHANCED FOLLOW-UP SERVICES (EFS) PROGRAM All clients All clients will receive intensive follow-up services for up to 30 days. This includes (at minimum): Contact within 24 hours of receipt of referral Weekly contact with Follow-Up Specialist Each contact Each contact will include the following: Risk assessment will be completed Crisis Management Plan review/development Means restriction education Referral/Linkage to additional needed services TARGET clients TARGET clients (ages 25-64) will receive additional contact with Follow-Up Specialist for up to one year in the form of a phone check-in at months 3/6/9/12.
