SOAR Model • SOAR Case Managers are trained on the SSI/SSDI application process and develop partnerships with SSA and DDS staff • Serve as appointed representatives for clients, collecting medical records, and verifying necessary information • Submit complete and high quality applications and track process status for clients  4

  5. 2017 SOAR Outcomes • 64% national approval rate for initial applications with 73% approval rate in DC • 28.5% approval for all non-SOAR applications • 12% approval for non-SOAR applications from individuals experiencing homelessness and serious mental illn ess • 96 day national average for SOAR approval • Over a year for non-SOAR applications • Over $356 million in benefits for SOAR clients  5

RFA Overview • Name: Supplemental Security Income (SSI) / Social Security Disability Income (SSDI) Outreach Access and Recovery (SOAR) Advocacy • Purpose: Identify Grantee(s) with plans to utilize the SOAR methodology to work with clients, medical providers, and SSA/DDS staff to submit applications, provide follow-up, and ensure communication to improve outcomes and shorten process timeframe • Funding: Up to $500,000 for up to five providers • Award period: Sep. 28, 2018 to Sep. 27, 2019 – Up to three additional option years  6

  7. Target Population • Eligible adults who – Live in the District, – Are experiencing homelessness, at risk of homelessness, or in a supportive housing program, and – Have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder  7

  9. Program Overview • Increase access to SSI and SSDI benefits for the target population; • Develop collaborative relationships with the local SSA field offices and DDS personnel; • Work with the referral sources, DHS, and community partners to identify potential candidates;  9

  10. Program Overview Con’t . • Assess individuals who have potential eligibility for SSI/SSDI; • Serve as the client’s appointed representative; • Collect medical records from providers who have treated the client; • Write a comprehensive SOAR Medical Summary Report (MSR);  10

SOAR Performance Standards • 100% of the funds from this grant are for SOAR-specific activities • 50% of SOAR Case Managers time must be dedicated to SOAR • Submit a minimum of 20 SSI/SSDI applications via SOAR – 50% of the applications for DHS supportive housing clients • Submit the Appointed Representative form within 30 days of accepting a client • 75% of the applications should be submitted within 55 days of the protective filing date • Submit medical records/MSR within 30 days of claim transfer from SSA to DDS • Use the SOAR Online Tracking Application for all SOAR applications and document all relevant client information in HMIS  12

Program Budget & Narrative • Proposed salaries and wages for project staff • Proposed benefits comparable to those paid to the other staff (show fringe rate) • Proposed supplies and materials • Rental or leasing of space for SOAR Advocacy – Rents must not exceed prevailing rates – Include utilities, telephone, maintenance services directly related to project activities – Include insurances, subscriptions and postage • Indirect: Show calculation and indirect rate  15

  18. Application Format • 8½ by 11-inch paper • Margins must be no less than 1 inch • 12-point font (Times New Roman recommended) with 1.5 line spacing • Pages double sided and numbered • Review panel will not review applications that do not conform to these requirements  18

  20. Capacity & Experience (35 Points) • 15 Points: Knowledge and experience relevant to providing SOAR Advocacy • 10 Points: Appropriateness and cultural competency of services to be delivered • 10 Points: Qualified staff with the training and experience to offer SOAR Advocacy  20

Program Design (40 Points) • 5 Points: Proposed activities/work plan will result in timely project start-up • 35 Points: Describe how the SOAR Advocacy model will: – Develop relationships with SSA and DDS – Identify and assess potential clients – Serve as appointed representative and engage clients throughout the process – Submit complete and accurate applications – Track and report application outcomes  21

  22. Fiscal Management (20 Points) • 10 Points: Provide evidence of sound fiscal management and financial stability • 10 Points: Demonstrate that the proposed budget is reasonable, realistic, and will meet performance standards  22

  23. Overall Feasibility (5 Points) • 5 Points: Provide documentation that the proposed program will be fully supported by management and that the project is compatible with the mission of the organization and will be effectively coordinated and integrated with its other activities  23

  24. Application Deadline • To be considered for funding, Applications must be received no later than 4:00 p.m. on July 30, 2018 • Supplements, deletions or changes to the application will not be accepted after submission  24
